to the Reading School District
Intervention School Counselor’s Page. Intervention Counselors
provide services that are available to all students and are designed
to compliment the existing educational program. We promote and
support the student’s academic and social success, and advocate for
and assist students to accomplish tasks associated with their
learning, growth and development.
Intervention
Counselors demonstrate the ability to maintain effective
relationships with students and families, in partnership with the
school system and the community. We identify specific needs, create
and implement a success plan based on needs and work in conjunction
with the Supervisor of Student Services, School Principal and other
school personnel. We are committed to the importance of continuous
school improvement that contributes to appropriate school and
learning environments.

Intervention
Counseling Services
When to
refer students who may require services from an Intervention
Counselor
When they
show signs of social or emotional difficulties that interfere with
their adjustment and/or academic achievement in school such as:
Ø
Students do not
appear to have any social relationships.
Ø
Students do not work
up to their known ability.
Ø
Students neglect to
do assigned school work.
Ø
Students are
disruptive to the school environment.
Ø
Students are
frequently absent or tardy.
Ø
Students do not
respect the rights of others.
Ø
Students regularly
complain of not feeling well.
Ø
Students are
excessively quiet, shy or withdrawn.
Ø
Students need to
improve problem solving and decision making skills.
What do
Intervention Counselors Do?
Ø
Provide crisis
intervention services to students and families.
Ø
Provide group and
individual counseling services to students.
Ø
Represent the
Reading School District on numerous committees such as: Gang
Reduction Intervention and Prevention Education (GRIPE), Safety
Committee, Rapid Response Team.
Ø
Address issues
concerning academics, personal safety, harassment, truancy,
bullying, problem solving, anger management, impulse control,
friendship, stress management, social skills, coping skills and task
management to all referred students.
Ø
Collaborate with
families to ensure utilization of needed services is a joint effort.
Ø
Refer families to
community agencies for material needs such as food, clothing and
medical assistance, and consult and coordinate with community
agencies also working with students to ensure continuity in the
student’s overall program.

Bio Information
Mr. Jacobs joined the
Reading School District as an Elementary School Counselor in 2005.
He has over nineteen years of Pennsylvania Civil Service experience
in law enforcement, psychology and school counseling. Mr. Jacobs
served honorably in the US Army 4th Infantry signal
Corps, Colorado, until 1992. Mr. Jacobs has completed the Student
Assistance Program (SAP) training, the Highmark Anti-Bullying
training and he served on the American School Counselor Association
Committee.

As an intervention
counselor for Reading High School, Barbara Perez generally provides
one to one counseling; conduct small and large group sessions with
the at-risk population. In addition she consults with school
administrators, teachers, school counselors, and other educational
professionals. In collaboration with the above professional staff,
strategies are developed and implemented to promote positive and
productive lifestyle choices. Services such as on-going support,
connection with personalized resources as well as family involvement
are provided.

worked with children and families for the past 30 years. He has

and Secondary School Counselor. He has worked in the field of
education for the past 16 years within Berks County schools.

Mrs. Daye-Barton
was a Behavior Specialist and a mobile therapist who worked within
the Pennsylvania Wrap around System for the past six years before
joining the Reading School District in August 2007. Mrs.
Daye-Barton is trained within the discipline of Post Traumatic
Stress Disorder, Anxiety, Depression, Anger Management, Coping
Skills, Mentoring and providing individual and family therapy.

Information
Mrs. Fake has
worked in the Mental Health field as Therapeutic Staff Support,
Student Assistance Program Liaison, Mental Health Technician,
Learning Support Assistant and Outpatient Therapist from 1988 until
August of 2007. At that time she was hired as an Intervention
Counselor for the Reading School District, where she currently
serves students at Southwest Middle school.
Mrs. Fake is
a member of the American School Counselor Association (ASCA) and the
Berks Area School Counselor Association (BASCA). Mrs. Fake works
hands on with students at-risk, with attendance issues, academic
concerns, and behavior problems. She works in conjunction with the
Home-School Visitor, Principal, Vice Principal and School Counselor.

Prior to working at
Reading High School as an Intervention Counselor, Mr. Gilmore worked
in the mental health field as a Behavioral Specialist and Mobile
Therapist providing intensive individualized therapeutic
interventions to families and schools.
Mr. Gilmore has
established and organized the Iron Men Association whose mission to
promote and support at-risk youth. He also participated in the
Learning for Life Program at the Berks County Juvenile Detention
Center. |
