USE OF SCHOOL FACILITIES

• Make sure that your name, address & phone number are on the back of the application form.  Without this information your application can NOT be returned to you when approved or denied.

• If your group is under a category that requires Liability Insurance, a copy of the liability certificate must be sent along with your application.  The application will NOT be processed without this certificate.

• Use of school equipment in conjunction with the use of the facility must be specifically requested on this application – NOT after your application is approved.  If you have any questions regarding equipment usage, please call Grace at 610-371-5667 and she will assist you.

• All bills for facility usage will be sent out by the Business Office after the completion of your rental.

• Applications can be returned either through the inter-school mail, faxed, or by the postal service to:
