The date for Winter Sports Physicals is October 30th, 2025
Times are as follows:
All Middle School Sports: 5:30 PM – 6:30 PM
HS Cheerleading & Swimming: 6:30 PM – 6:45 PM
HS Boys Basketball: 6:45 PM – 7:00 PM
HS Wrestling & HS Bowling: 7:00 PM – 7:15 PM
HS Air Rifle & HS Indoor Track: 7:15 PM – 7:30 PM
HS Girls Basketball: 7:30 PM – 7:45
Any Fall athletes who competed for Junior High or High School teams do not need to get a
new physical. They must simply complete the Section 7 re-certification form.
All students wishing to receive a sports physical must have their student account and parent
account complete sections 1 through 5 and St. Luke’s forms on Planet HS
ALL FORMS MUST BE DONE NO LATER THAN OCTOBER 24TH TO GET A SPORTS PHYSICAL
